Exploring the Influence of Age on Tympanoplasty: Closure Rates and Hearing Outcome

Introduction: The objective of this retrospective study is to investigate how age may affect tympanoplasty closure rates and hearing outcomes.

Methods: A retrospective chart review was conducted on patients at a tertiary care center undergoing tympanoplasties for tympanic membrane perforations without ossiculoplasties or mastoidectomies between 2016 and 2021. Audiological data, including pure tone average (PTA) and word recognition score (WRS) following the AAO-HNS guidelines, were extracted for analysis.

Results: A total of 90 patients (51% females) with a mean age of 49 ± 22 years were included. Patients were dichotomized by the mean age and had similar smoking status (p = 0.8), gender (p = 0.7), body mass index (p = 0.2), and race (p = 0.8). Older patients had a higher incidence of diabetes (p = 0.02) or obstructive sleep apnea (p = 0.04). Rates of closure at three months were similar between older patients (85%) and younger patients (95%) (p = 0.1). Among the young patients (n = 43), the post-treatment scattergram showed that 25 (58%) patients had PTA improvements of >11 dB, 2 (5%) had no change, and 5 (12%) had worse PTA. Additionally, 7 (16%) patients had varied improvements in WRS scores. Among the older patients (n = 47), the post-treatment scattergram revealed 19 (40%) patients with PTA improvement of >11 dB, 2 (4%) with no change, and 11 (23%) with worsening PTA. Furthermore, 17 (36%) patients demonstrated varied WRS score improvements.

Conclusions: While closure rates were comparable between age groups, differences in hearing outcomes were observed. The comparison revealed a higher percentage of PTA improvement in younger patients, while a higher portion of older patients experienced worsening PTA.
