Audiological Outcomes of CMV-Positive Newborns in Support of Universal Screening Development

Introduction: Cytomegalovirus (CMV) is a prevalent congenitally acquired infection, affecting 0.2-2% of births. Even when asymptomatic at birth, CMV-positive newborns may develop sensorineural hearing loss (SNHL) and developmental delays. This study aims to investigate the audiological outcomes of CMV-positive newborns identified in an universal screening program.

Methods: Preliminary data of newborns admitted to three Methodist Nurseries and LeBonheur Children’s Hospital NICU in Memphis, TN between 3/28/2016 and 01/16/2023 were screened using infant saliva specimens collected within two weeks of birth. Specimens underwent central processing at LBCH molecular laboratory using real-time CMV PCR (SimplexaTM CMV). Positive cases underwent evaluation, including CBC, CMP, head ultrasound, ophthalmology (described elsewhere) and audiology referral. The study received institutional IRB approval, and IBM SPSS statistics statistical analysis was conducted.

Results: The study included 209 newborns, 110 males (52.1%) and 99 females (46.9%). Audiological assessments revealed that 96.7% of the newborns underwent hearing screening, of which 82.9% passed. Among those who passed, 90.0% exhibited normal hearing in the right ear, and 87.5% in the left ear. In the overall cohort, 91.5% had bilateral normal hearing, while 2.8% presented with congenital hearing loss. The SVI had a mean value of 0.386. The average age of hearing loss detection was 6.67 months for the right ear and 6.42 months for the left ear. Notably, there was not statistical significance between CMV urine viral load by PCR in patients with and without hearing loss (p-value 0.87). Furthermore, out of the 50 detected patients who received antiviral therapy, only 3 required hearing aids (6%), and 4 needed cochlear implants (8%), suggesting potential efficacy of the intervention.

Conclusions: This study highlights the significant impact of congenital CMV infection on hearing in newborns, even when asymptomatic at birth. It emphasizes the need for universal screening programs to identify CMV-positive newborns and their potential risk of sensorineural hearing loss.
